We see patients age 16 and up for common illnesses and complex chronic medical problems, and are devoted to ensure longevity of our patients' health and developing a long-lasting relationship with them.

Dr. Adnan Seljuki
Internal Medicine Specialist
Dr. Adnan Seljuki is board certified in Internal Medicine. He attended Khyber Med College and completed his residency at New York Methodist Hospital. He is an independent physician, but is affiliated and sees his patients at Decatur Morgan Hospital.
Dr. Seljuki believes practicing good medicine means more than just seeing the doctor when your sick. He likes connect with patients and form a treatment plan that best fits you. Good medicine involves providing preventative approaches to keep you healthier for longer.
Dr. Seljuki has enjoyed practicing in Decatur for the last 15 years. Outside of work, he enjoys spending time with his family and traveling. |
Candice Blackwood
Family Nurse Practitioner
Candice joined our practice in April 2023. Candice graduated with a bachelor of science in nursing from University of North Alabama in 2016. She worked as a RN, in the critical care unit at Huntsville Hospital while obtaining her masters of science in nursing. While in this position she prided herself on being a leader and charge nurse. She was awarded AICU Nurse of year 2020. Candice is a Board Certified Nurse Practitioner. After receiving her Masters in Nursing she worked as a Family Nurse Practitioner for Lawrence Medical Center, and Urgent Care. She worked there for 2 years. Candice has evaluated and managed acute conditions exacerbations of chronic illness, as well as a complex, multi-system medical disease.
Candice is excited to get to know each patient individually. She strives to be a partner in her patient’s wellness, health promotion and disease management. When Candice is not treating patients, she enjoys reading, and playing with her golden- doodle. She also enjoys spending time with her husband, family and friends. |

Hannah Long
CRNP
Hannah joined our practice in November 2022. Hannah earned her Associate's degree in Nursing in May of 2014 from Northwest Shoals Community College. She worked full time while obtaining her bachelor’s degree in nursing. Hannah chose to continue her education and earn a Master’s Degree in 2019.
Hannah is a Board Certified Nurse Practitioner. She is a member of the North Alabama Nurse Practitioner’s Association and the American Association of Nurse Practitioner’s. Hannah is very educated and comes with years of medical experience. Hannah worked at Lawrence Medical Hospital as a RN, CRNP and served as the ER director. She worked at Lawrence Urgent Care for 2 years. Hannah has experience with acute illness, chronic conditions, and emergency medicine.
Hannah strives to be a partner in her patient’s wellness, health promotion, and disease management. Hannah enjoys getting to know each patient personally. When Hannah is not treating patients she enjoys spending time with her husband, daughter, family and friends. |
