Office Hours & Appointments

Patients are seen by appointment time, and not arrival time. We do try to accommodate walk-in’s but the patients with appointments will be seen first. Patients arriving more than 10 minutes late will be asked to reschedule. If we are able to accommodate the patient, they will be considered a walk-in.

Office Hours: Monday - Thursday 8am - 5pm
We are closed from 12pm -1pm for lunch each day.

Patient Forms & Downloads

For your convenience, you may complete new patient forms in advance. Please select and print from the options below, fill them out completely and bring them with you to your appointment. If you are not able to complete the forms before your visit, please allow at least 30 minutes for completion in the office before you see the doctor. We recommend that you:

  • Bring your insurance card.
  • Bring all current medications in original bottles.
  • Please arrive a few minutes early to fill out paperwork. At your request, we can also send you our New Patient Information Packet in advance to fill out and bring to your first appointment.
  • Bring a copy of any important prior records if you are transferring your care from another doctor.
  • Note that if a co-pay is required by your insurance company, it is due at the time of your visit and may be paid in cash, by check or with a credit card.

  • Patients are asked to bring medications at each office visit.
  • Medication refills are not given to patients who do not keep their follow up appointments.
  • Medications will not be prescribed for illness unless the patient sees the physician first.
  • Patients must allow 24- 48 hours for prescription refills.
  • The physician “on-call” does not refill medications.

Insurance Providers

If you are not insured by a plan we do business with, payment in full is expected at each visit. If you are insured by a plan we do business with, present your current card at your office visit so that we may verify your coverage. Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have regarding your coverage. If you have questions about whether we accept your insurance provider, please call (256) 309-5850 to speak with one of our staff members.

Payment & Fees

  • Payment is Due at the time of service. This includes co-pays, co-insurance, deductibles, and previous balances. Payment for services is the responsibly of the patient.
  • Patients are charged $30.00 for appointments no cancelled within 24 hours of seeing the provider.
  • Patients are charged $ 50.00 for appointments not cancelled within 24 hours prior to a scheduled test. (Ultrasound, Echo, Carotid Duplex)
  • Forms such as FMLA, Disability, etc. are subject to a $25.00 fee. The fee is required to be paid before forms are faxed, or picked up. We require no less than 10 days for preparation.
  • After hour calls that are non-emergent are subject to a fee at the providers discretion.
